Instructions - There are no download restrictions or fees. Print, laminate, copy and distribute these forms freely to any healthcare providers who can use them.

The Adobe PDF files can be downloaded in two forms: one with pronunciations (e.g., 'kwan-tohs ahn-yohs'), or another without pronunciations for those that feel comfortable with the language. The individual assessments are downloaded individualy in order to seperate them as printable files and in the reader menu on PDA devices.

To view the full document on your desktop simply download the PDF icon and view it with any version of Adobe Acrobat. To get the free Acrobat Reader click on the Adobe icon ot the bottom of the page.

The audio files download via a compressed MP3 format which can then be either listened to on a computer as an MP3 file, or converted to a audio CD in the preferences section of most audio programs under 'Burn As - Audio CD.'
